Abstract
Background: Educational methods on simulation are variously applied to medical education. But computer-based simulation has not been defined to apply student assessment especially in comprehensive clinical diagnosis. The aim of this study was to evaluate a computer-based simulation program applying to test for medical students.

Methods: 70 third-year medical students had experienced 2 times computer-based simulation with orientation and feedback. For 2 weeks, they had simulation on cardiac arrhythmia, chest pain, airway & breathing, cardiac arrest simulation each 2 cases, and then they answered the questionnaire. Their genders, related subjects¡¯ scores, and computer skills, level of experience on online games and similar simulated programs were analyzed with the scores of computer-based simulation. And the results of questionnaires were analyzed.

Results: The scores of computer-based simulation were not statistically different between male and female students. There was not correlated between the scores and the related subjects¡¯ scores (Pearson¡¯s r=-0.001, p=0.994). Computer skills were statistically different between them (t=2.544, p=0.013) and related with the scores of computer-based simulation (Spearman¡¯s r=0.318, p=0.011). The students¡¯ answer was positive response to the usefulness of computer-based simulation such as whether that gave the opportunities of integrating detailed medical knowledge, giving total understanding process of treating patients etc.

Conclusions: Computer-based simulation program on comprehensive clinical diagnosis is useful for medical students to learn clinical reasoning. But to apply it to the test them, we give them much experiences on various simulated program especially comprehensive and integrated clinical contents.
